The form on page four can be filled out electronically or by hand, printed, signed, and dropped off at any Blackhawk Bank & Trust location or mailed to Blackhawk Bank & Trust, PO Box 1100, Milan, IL 61264. Applications are also available at any branch or can be mailed upon request by calling our Bookkeeping Department at 309-787-7567.
